You’ll practice within an employer-sponsored clinic, caring for a consistent patient population — not a high-volume, traditional setting. This allows you to work with autonomy while still being supported by Health Bar’s broader clinical team.

This is a proactive, employer-based care model designed to support long-term health outcomes.
What You’ll Do- Provide onsite advanced clinical care, including acute, episodic, preventive, and chronic disease management
- Deliver proactive care for employees, spouses, and dependents
- Collaborate with HPP nurses on claims analysis and care coordination
- Establish clinical pathways and treatment protocols
- Conduct telehealth visits as appropriate
- Coordinate care with external provider networks
- Maintain required licensure and certifications
- One 8-hour shift, any day Monday through Friday
- Starting at $55/hour, based on experience
You’re looking to complement another role or maintain a light, consistent weekly schedule. You enjoy practicing independently, focusing on prevention and chronic care, and building real relationships — all within a manageable time commitment.
Required Qualifications- Master’s degree in Nursing from an accredited program
- Active, unrestricted Michigan RN license
- Active, unrestricted FNP certification (AANP or ANCC)
- Active Michigan NP Specialty Certification
- 2+ years of nursing experience (med-surg, ER, ICU, acute care, public health, school nursing, or outpatient preferred)
- BLS certification (AHA or Red Cross) within 90 days of hire
- Must be authorized to work in the U.S. without employer sponsorship
The pay range for this role is:
55 - 65 USD per hour (Traverse City, MI)
