Responsibilities
  • Lead and collaborate to manage and deliver patient care plans for home health patients.
  • Provide quality patient care and ensure timely charting and documentation.
  • Self‑manage time and schedule, dedicating roughly 20% of the day to travel between patient locations.
  • Utilize tablets, mobile phones, and EMR software for patient information.
  • Participate in a 4‑month orientation with pay guarantee, and receive monthly clinical outcome bonuses after one year.
Qualifications
  • Valid RN license in the state of practice.
  • Graduate from an approved school of practical nursing.
  • One year of RN experience and clinical competence in delivering patient care.
  • Current driver’s license and ability to travel ~20% of the day to patient locations.
  • Commitment to meet critical charting deadlines consistently.
  • Experience with tablets, mobile phones, and EMR software.
  • Successful completion of a drug screen; background check includes medical and recreational marijuana.
