The ideal candidate will join a team of experienced, collaborative, and proactive healthcare providers, clinicians, and support staff dedicated to delivering high-quality, patient-centered care to the patients and families living with complex, serious illness.
PositionDuties/Highlights Include
- Full-time position
- Monday – Friday schedule
- 36 hours of direct patient care per week
- Provide expert management of complex symptoms in patients with serious, life-threatening, and life-limiting illnesses, including pain, dyspnea, nausea, anxiety, and depression
- Deliver meticulous, evidence-based symptom control with attention to patient comfort and quality of life
- Engage in clear, compassionate, and effective communication with patients and familiesregardingserious illness, prognosis, goals of care, and treatment preferences
- Collaborate within an interdisciplinary team to support holistic patient care, including psychosocial and emotional needs
- Coordinate complex care across healthcare settings, ensuring continuity and alignment with patient goals
- Master’s Degree in Nursing OR Master’s Degree in Physician Assistant Studies
- State of Michigan Nurse Practitioner or Physician Assistant licensure required
- Active DEA registration required, with the ability to prescribe controlled substances in accordance with federal and state regulations
- Strong fund of knowledge in managing complex symptoms associated with serious illness
- Foundational expertise in the management of pain, shortness of breath, nausea, and mood-related symptoms including anxiety and depression
- Demonstrated skill in serious illness communication with patients and families
- Ability to coordinate complex, multidisciplinary care within healthcare systems
