Overview
Under the direction of the Senior Director of Behavioral Health, the Health Connections Assistant (Ameri Corps VISTA) aims to improve patient access to community resources, address social determinants of health, and enhance the overall well‑being of HJAHC patients. The VISTA member will collaborate with various stakeholders to implement and evaluate the impact of the Health Connections volunteer program, identify and address barriers to patient access.
The VISTA member will also contribute to expanding the program's reach and impact through outreach and marketing efforts.
- Support and update workflows, policies, and procedures for the Health Connections program.
- Develop strategies to reduce barriers that limit patient access to community resources.
- Provide on site and virtual support to volunteers, working with university partners and staff to resolve concerns.
- Track and document volunteer activities and patient encounters accurately.
- Maintain and expand a current directory of community resources.
- Escalate patient concerns to clinical or management staff when needed.
- Assist with volunteer recruitment and patient outreach using approved materials.
- Collect and review monthly data, present recommendations for program improvement, and ensure confidentiality, professionalism, and compliance with safety guidelines.
