This full-time on-site role as a Nurse is based in Trenton, NJ. The role involves delivering high-quality patient care in accordance with individualized care plans, performing health assessments, and administering medications. Additional duties include coordinating with healthcare professionals, educating patients and families about care plans, and maintaining accurate patient records. The role requires close collaboration with the care team to ensure the best possible outcomes for patients while fostering compassionate and patient-centered services.

• Demonstrated skills in patient care, health assessments, and medication administration
• Competence in care coordination and collaborating with interdisciplinary healthcare teams
• Effective communication, patient education, and counseling skills
• Experience with patient records management and documentation
• Active Registered Nurse (RN) license in the state of New Jersey
• Basic Life Support (BLS) and Certified CPR credentials
• Strong organizational skills and attention to detail
• Experience in community health or home healthcare is a plus
• Passion for compassionate care and improving patient outcomes
