Naven Health delivers home infusion services for a broad range of specialized therapies, as well as clinical trial services and special programs for pharmaceutical manufacturers.
An Infusion Nurse I is a licensed registered nurse who provides basic direct patient care to patients in the home or an alternate infusion site, ensuring patient safety under a physician’s plan of care.
Responsibilities- Initiate, develop, and implement the basic nursing plan of care treatments, evaluating patient progress toward goals.
- Participate in direct patient care, performing treatments, administering medications, and educating patients and families/caregivers.
- Modify the plan of treatment in response to changing patient status based on physician orders to achieve established or revised patient care goals.
- Assess patient needs and physical status at each visit through health data access and patient interview.
- Re‑evaluate patient needs through physical reassessment, response to therapy, and supplemental physician orders.
- Obtain and clarify physician orders for treatment revisions, inform physicians promptly of significant changes in the patient’s condition, and provide written summaries within supplemental orders.
- Provide effective and safe teaching using a patient‑centered care approach for patients and families to achieve independence with prescribed therapy and care needs.
- Effectively and timely communicate with Naven Health’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care.
- Prepare clinical documentation in real‑time during visits and submit to the nursing department to comply with established timelines for billing optimization.
- Under close supervision, coordinate discharge planning and prepare discharge summaries with patient instructions, thoroughly reporting patient care needs, progress, and goals when transferring care.
- Demonstrate compliance with agency operations, Naven Health’s policies and procedures, professional standards, local, state, and federal regulations/guidelines, and accreditation standards.
- Maximize work efficiency through the use of computers and other automation technologies to validate treatment orders, communicate patient care provided, and follow the assigned visit schedule.
- Respond promptly and appropriately to patient requests.
- Initiate emergency procedures as necessary.
- Accept accountability for own practice through ethical and professional conduct.
- Follow established programs and practices within policies and procedures reflective of Naven Health’s mission, values, and objectives.
- Observe legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Naven Health information including adherence to HIPAA regulations.
- Speak knowledgeably about Naven Health’s scope of services and effectively instruct patients about the related plan of care.
- Attend and complete required training modules, in‑services, and continuing education to maintain competency and professional licensure regarding the care and management of patients in the home and/or alternate care settings.
- Effectively provide oversight and coordination of paraprofessionals in the home setting and may act as the supervisor in the absence of the supervisor or nurse manager, as applicable under state and federal regulations.
- Participate in multidisciplinary team conferences and provide precepting, training, and mentoring to other nurses for orientation, onboarding, and supervisory activities as assigned.
- Participate in nursing on‑call responsibilities as applicable.
