Registered Nurse, RN

• Create and manage individualized plans of care
• Create care plans for LPNs to follow
• Conduct check-ins with individuals at the group home
• Provide medical care and monitor and follow through on care
• Provide medication education and assist with medication administration and documentation
• Encourage normalization and independence of individuals served
• Train and assist individuals served and staff on medical education and related areas
• Liaise with outside medical providers to coordinate care
• Maintain accurate written medical, dental, and related records
• Intervene in emergencies involving individuals served
• Attend interdisciplinary team (IDT) meetings
• Oversee Licensed Practical Nurses (LPNs)

• Currently Licensed Registered Nurse (RN) in NJ
• Excellent interpersonal and client care skills
• Valid driver’s license
• First aid/CPR certification required
• Ability to build rapport with patients
• Experience working with individuals with developmental disabilities preferred
• Availability Mondays and Tuesdays, 7am–7pm

Demonstrates expertise in creating and managing individualized care plans, providing medical care, and training staff and individuals served. Strong ability to coordinate with medical providers and maintain accurate medical records while ensuring client independence and normalization.
