Position: Licensed Practical Nurse (LPN) – Dayshift
Type: Regular

Schedule: 7:00 AM - 7:00 PM
Location: Trenton, TN (On-site)
Compensation: $30.42
Incentives: No sign‑on bonus offered; no relocation package offered.
Job Responsibilities- Provide holistic nursing care to individuals with intellectual and developmental disabilities
- Assist the Registered Nurse in delivering quality nursing services to persons supported
- Provide health‑related training to unlicensed personnel
- Administer medications and treatments or assist the person supported with medication administration
- Assist with ensuring preventative and routine medical appointments are scheduled
- Conduct medication closest assessments
- Collect health‑related data
- Observe for and report any abnormal health‑related findings to the physician
- Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team
- Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported
- Graduation from an accredited licensed nurse program
- Current nursing license with no restrictions to practice nursing
- Current CPR certification or eligibility to obtain certification
- Must be able to communicate effectively with Interdisciplinary Team members, the people supported, and their families
- Must be able to work independently, be organized, and possess the ability to prioritize work tasks
- Regularly lift up to 40 pounds (including 20-pound lifts)
- Pull up to 25 pounds
- Squat, kneel, crawl, crouch, climb, and stoop as needed
- Regularly stand and walk
- Demonstrate proficiency in CPR from the floor
- Meet vision requirements: close, distance, and peripheral vision
- Communicate effectively
- Complete criminal background check
- Name checked in registries (OIG exclusions database, Child Abuse Registry, Offenders Against Individuals with Developmental Disabilities)
- Drug testing
- Education verification and other credentialing based on position requirements
- Proof of employment history or references (if required)
- For positions requiring driving, proof of driver's license, insurance, and vehicle if necessary for transportation
