Job Overview

Registered Nurse (RN) – Hospice Admissions, located in Troy, Michigan.

We are seeking a compassionate nurse to support patients in Troy and surrounding areas. This position is full‑time, 40 hours per week, Monday through Friday, 4:00p.m. – 12:00a.m. The role includes management and delivery of patient care plans, charting, and coordination with the care team.

• Collaborate with the Hospice Admissions team to manage and deliver patient care plans.
• Maintain accurate, timely charting and documentation.
• Self‑manage time and schedule, ensuring critical deadlines are met.
• Conduct patient care visits, including driving to/from patient locations up to 20% of the day.

• RN license in the state of employment.
• Associate degree or higher from an accredited School of Nursing.
• Minimum two years of RN experience; hospice experience preferred.
• Current driver’s license and ability to travel to patient sites.
• Experience with tablets, mobile phones and EMR software.
• Commitment to meet critical charting deadlines.
