At Residential Home Health and Hospice, we’re building an exceptional care team. Our home‑health registered nurse case managers lead and collaborate to deliver patient care plans within a team‑based environment.

• Lead and collaborate to manage and deliver patient care plans.
• Maintain accurate and timely charting of patient care.
• Support patients in Brownstown, Allen Park, south to the MI/OH Border and surrounding areas.
• Participate in a 24/7/365 after‑hours care team as needed.
• Utilize tablets, smartphones, and EMR software for documentation and communication.

• Registered Nurse (RN) license in the state where you will work.
• Graduate from an approved school of practical nursing.
• At least one year of RN experience and clinical competence in patient care.
• Valid driver’s license and ability to travel up to 20% of the day to patient locations.
• Commitment to meet charting deadlines consistently.
• Proficient in self‑management of time and scheduling.
• Experience with tablets, mobile phones, and EMR software.
