• Must have a valid nursing license with verification dated within 30 days of start.
• American Heart Association BLS certification required for all direct patient care personnel.
• ACLS certification required for RNs in critical care areas including PCU.
• Completion of Trinity Specialty Specific Competency Testing within the last year.
• Completion of Trinity Skills Checklist for clinical contractors.
• Must provide documentation of immunizations and health screenings including Fit Test, Trinity 10 Panel Drug Screen, MMR, Hepatitis B series, Influenza Vaccine (optional), 2 Step PPD or IGRA, Varicella, Physical/Health Statement, Tdap vaccine or declination if no pediatric interaction.
• Completion of various Trinity professional and compliance forms including Code of Conduct, I9, Confidentiality Agreement, Integrity and Audit Services Form, Conflict of Interest Disclosure, Criminal Background Check, Education Verification, EPLS, GSA/OIG, Joint Commission Testing, NSO, and System Access Form.
