Schedule:

Monday - Friday. 8:00 am to 5:00 pm with Holiday rotation

** HOW YOU'LL WORK:
** You’ll provide support and act as a resource to clinicians in the field. You will assist in monitoring compliance quality and safe delivery of services by following a clinical process set by clinical management.
** MAJOR AREAS OF RESPONSIBILITY:**
* * Operations:
* Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concerns.  Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to manager.  Monitor Home Health Aide supervisory visits.
* * Patient Care:
* Work in the field with staff and patients as needed.
* Customer Service:
Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.
* * Minimize Patient Risk:
* Ensure Start of Care compliance and quality and safe delivery of home health services. Focus on Outcome Monitors as assigned.  Implement quality improvement programs in the field.
* * Policies:
* Review clinical documentation and maintain current knowledge of all Medicare/ Federal/ Insurance / Company and Hospital guidelines, policies and protocol.
* * Teamwork:
* Assist with monthly Case Manager, Nurse and Therapy meetings.  Participate in all on-call requirements, and patient staffing as needed.

** REQUIREMENTS:
** Minimum one-year home health experience is preferred
* Current RN license in the state of employment
* Minimum of Three (3) years of experience as an RN in an acute care setting
* Participates in on-call, evening/weekend shifts and provides patient care, per agency needs
* Valid driver's license and auto insurance in your name as a driver
* Capable of all physical demands
