• Conduct initial and ongoing assessments of patient needs
• Implement and adjust individualized care plans
• Ensure compliance with hospice policies and Medicare regulations
• Educate patients and families on self-care and end-of-life options
  
  Supervise paraprofessionals delivering patient care
• Document and report clinical findings and participate in interdisciplinary meetings
• Enhance patient satisfaction and experience

• Current Michigan LPN License in good standing
• Minimum of 1 year experience in hospice or palliative care
• Strong empathy and communication skills
• Flexibility to respond to varying patient needs
