Schedule:

Mon-Fri (8 AM - 5PM) on call requirements

Beaumont Home Health is seeking a compassionate and dependable LPN to provide skilled nursing care to patients in their homes.

What You’ll DoProvide skilled nursing care according to physician orders and RN care plans

Monitor patients and report changes to the RN Administer medications and treatments as prescribed

Educate patients and families on care and disease management

Complete accurate, timely documentation

Communicate with patients, families, and the care team Participate in on-call responsibilities, meetings, and training

Travel to patients’ homes within the assigned service area

Requirements

Current LPN license in the state of employment1+ year of acute care LPN experience required2 years preferred; home health experience a plus Strong communication, organization, and clinical judgment

Compassionate and patient-focused

Valid driver’s license and auto insurance
