Job Description
Summary:
The Housing Resources Coordinator plays a critical role in the implementation of the diversion grants to reduce homelessness through integrated shelter diversion and housing navigation services. This position supports individuals and families experiencing a housing crisis by helping them avoid shelter entry or rapidly exit shelter into stable housing. As a full-time position you will be working a schedule of 40 hours per week between the hours of 8:30 am to 4:30 pm, Monday through Friday.
Essential Functions:
- Conduct diversion screenings and intakes to assess immediate housing alternatives and prevent shelter entry by completing required paperwork and conduct a full assessment for the applicable grant funding and resources.
- Provide strengths-based case management services, including mediation and problem-solving to identify safe, viable housing options following Critical Time Intervention and Housing First models using a strength-based, person-centered approach.
- Assist individuals with housing search and placement, including identifying units, scheduling viewings, and completing applications, and linking individuals to community resources related to their unique household needs.
- Build and maintain relationships with landlords and property managers to expand housing opportunities and resolve tenancy issues.
- Execute all duties within the framework of Housing First and trauma-informed care principles.
- Administer flexible financial assistance to support housing stability, including rent, deposits, and other eligible expenses.
- Conduct follow-up case management services at 30-, 60-, and 90-days post-placement to ensure housing retention and connect individuals to ongoing supports.
- Maintain accurate and timely documentation in the Homeless Management Information System (HMIS) and other required databases.
- Demonstrate flexible and efficient time management and ability to prioritize workload.
- Consistently report to work on time and prepared to perform duties of position.
- Have a strong work ethic that equally prioritizes the needs of participants and the needs of the organization.
- Adhere to agency policies and procedures.
