Case Manager With The Community Housing Assistance Program (CHAP)

Are you passionate about advocating for individuals who may not be able to advocate for themselves? Join our team as a Case Manager with the Community Housing Assistance Program (CHAP) and help individuals experiencing homelessness access resources, stabilize their lives, and move toward permanent housing.

Key Responsibilities:

  • Build supportive relationships with participants experiencing homelessness.
  • Provide supportive counseling and daily living skills training.
  • Connect participants to community resources and assist with discharge planning.
  • Provide crisis intervention and assist with meeting basic needs.
  • Conduct home visits and provide transportation when needed.
  • Maintain documentation and case records.
  • Promote a culture of inclusion and belonging.

Qualifications:

  • Associate or Bachelor's degree in Human Services, Social Work, or a related field preferred (equivalent experience considered).
  • Experience working with diverse or high-needs populations preferred.
  • Valid NYS driver's license and access to a reliable vehicle required.
  • Bilingual candidates encouraged to apply.
