Position OverviewThe Social Work Case Manager works closely with the interdisciplinary team to ensure residents receive comprehensive, person-centered support. This role focuses on care coordination, documentation, resident engagement, and family communication.
Key Responsibilities- Assist with resident assessments and care plan development
- Support case management documentation and compliance
- Coordinate appointments and community resources
- Provide emotional support and advocacy for residents
- Communicate with families regarding care updates and services
- Assist with discharge planning and transitions
- Maintain accurate records in accordance with state regulations
- Participate in interdisciplinary team meetings
