Social Worker / Case Manager

Heartwood Assisted Living Community is seeking a compassionate, organized, and resident-focused Social Worker / Case Manager to support the emotional, social, and psychosocial well-being of our residents. This position serves as an advocate for residents and their families while coordinating services, facilitating care planning, and connecting individuals with appropriate community resources.

The ideal candidate is a caring professional with excellent communication skills, strong problem-solving abilities, and a passion for improving the quality of life for older adults.

Essential Responsibilities Resident Advocacy & Support
  • Conduct psychosocial assessments and identify resident needs upon admission and throughout residency.
  • Provide emotional support, crisis intervention, and counseling to residents and families as appropriate.
  • Assist residents with adjustment to assisted living and promote independence and well-being.
  • Advocate for residents' rights, dignity, and quality of life.
  • Support residents and families through transitions, changes in health status, and end-of-life planning when appropriate.
Case Management
  • Develop, implement, and monitor individualized service plans in collaboration with the interdisciplinary team.
  • Coordinate resident services with physicians, nursing staff, therapists, pharmacies, hospice providers, home care agencies, and community organizations.
  • Assist residents and families in accessing community resources, financial assistance programs, and supportive services.
  • Monitor resident progress and recommend modifications to care plans as needed.
  • Participate in resident care conferences and interdisciplinary meetings.
Family & Community Relations
  • Serve as the primary point of contact for psychosocial concerns.
  • Educate residents and families regarding available services, resources, and community programs.
  • Maintain positive relationships with residents, families, referral sources, and community partners.
  • Facilitate family meetings as needed to address concerns and care planning.
Compliance & Documentation
  • Maintain accurate, timely, and confidential resident records.
  • Ensure documentation complies with New York State Department of Health regulations and community policies.
  • Assist with Department of Health surveys and regulatory inspections.
  • Participate in quality improvement initiatives and maintain compliance with all applicable regulations.
Administrative Responsibilities
  • Maintain resident files, assessments, and case management documentation.
  • Prepare reports as requested by leadership.
  • Collaborate with Nursing, Activities, Dining Services, Maintenance, and other departments to support resident care.
  • Perform other duties as assigned.
Qualifications Required
  • Bachelor's degree in Social Work (BSW), Human Services, Psychology, Sociology, or a related field required.
  • Master's degree in Social Work (MSW) preferred.
  • New York State Social Work license (LMSW or LCSW) preferred but not required.
  • Minimum of two (2) years of experience in social work, case management, assisted living, long-term care, healthcare, or senior services.
  • Strong knowledge of community resources, discharge planning, and resident advocacy.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to manage multiple priorities while maintaining professionalism and confidentiality.
  • Proficiency in Microsoft Office and electronic documentation systems.
