Position Overview

Health Partners Free Clinic is a 501(c)(3) nonprofit who provides high-quality, compassionate healthcare to uninsured and underinsured neighbors in our community. We believe healthcare shouldn’t depend on your paycheck or your luck.

We are seeking a mission-driven Nurse Practitioner or Physician Assistant to join our team either as paid staff or as a volunteer provider. This role is ideal for someone who wants meaningful, relationship-based care, manageable patient volume, and the opportunity to make a direct impact in the lives of people who might otherwise go without care.

Responsibilities
  • Provide comprehensive primary care to uninsured and underinsured patients
  • Perform physical exams, patient assessments, and medical decision-making
  • Obtain and document medical histories and maintain accurate electronic health records
  • Develop, implement, and adjust individualized patient care plans
  • Order, interpret, and manage diagnostic tests and treatments
  • Prescribe medications and monitor patient response
  • Work collaboratively with a small, interdisciplinary team of providers, nurses, pharmacists, navigators, and volunteers to deliver coordinated, patient-centered care
  • Practice resourceful, high-quality medicine within a free clinic setting with limited financial and specialty resources
  • Partner with the care team to identify creative solutions that help patients overcome barriers such as cost, transportation, and access to medications
  • Educate patients and families on prevention, chronic disease management, and self-care
  • Refer patients to community partners and specialty services when care is not available on-site
  • Participate in quality improvement efforts and support a culture of compassion, dignity, and respect for every patient
  • Contribute to a positive, mission-driven clinic environment
Qualifications
  • Licensed Nurse Practitioner or Physician Assistant in Ohio
  • Prescriptive authority required
  • Experience in primary care or family practice preferred
  • Comfortable working with limited resources and diverse populations
  • Proficient with electronic health records and basic computer systems
  • Strong communication and teamwork skills
  • Understanding of HIPAA, OSHA, and regulatory compliance
  • Commitment to serving uninsured and underserved patients
