Registered Nurse – Transitional Care Coordinator

Overview

Assist the organization in living its Vision, Mission, and Values (integrity, patient‑centered, respect, accountability, compassion) while working under the supervision of the Nurse Manager/Clinical Coordinator.

Responsibilities
  • Assesses the patient’s physical and psychosocial condition continuously throughout the continuum of care.
  • Develops, communicates, and documents an individualized plan of care reflecting patient problems and interventions to achieve desired outcomes, involving patient, family, and health‑team.
  • Implements and coordinates the plan of care in collaboration with the health‑team.
  • Evaluates the effectiveness of the plan, patient response to care, and readiness for discharge.
  • Revises the plan as needed and makes referrals where indicated.
  • Plans education based on assessment of patient and family learning needs and readiness.
  • Provides education and documents outcomes.
  • Serves as preceptor for new staff.
  • Functions as charge nurse, delegating and supervising unit activities.
  • Meets performance expectations for customer service, teamwork, resource utilization, staff and self‑development as outlined in performance review.
  • Performs other duties as assigned or directed to ensure smooth operation of the department/unit.
  • Demonstrates competency in caring for neuro patients, including airway management, tracheostomy care, and assisting physicians during neuro procedures.
  • Performs pharyngeal, endotracheal, and tracheostomy suctioning according to established policy and procedure.
  • Uses equipment and cares for patients with a halo brace.
  • Manages ventilator‑dependent patient interventions.

Qualifications
  • Graduate of an accredited school of nursing; BSN preferred.
  • Proficient in using computer software, including electronic patient records.
  • Displays ability to use the internet for literature searches regarding patient care.
  • Valid Connecticut registered nurse license.
  • BLS required.
