Registered Nurse – Transitional Care Coordinator
Yale New Haven Health – Trumbull, CT
OverviewAssist the organization in living its Vision, Mission, and Values (integrity, patient‑centered, respect, accountability, compassion) while working under the supervision of the Nurse Manager/Clinical Coordinator.
Responsibilities- Assesses the patient’s physical and psychosocial condition continuously throughout the continuum of care.
- Develops, communicates, and documents an individualized plan of care reflecting patient problems and interventions to achieve desired outcomes, involving patient, family, and health‑team.
- Implements and coordinates the plan of care in collaboration with the health‑team.
- Evaluates the effectiveness of the plan, patient response to care, and readiness for discharge.
- Revises the plan as needed and makes referrals where indicated.
- Plans education based on assessment of patient and family learning needs and readiness.
- Provides education and documents outcomes.
- Serves as preceptor for new staff.
- Functions as charge nurse, delegating and supervising unit activities.
- Meets performance expectations for customer service, teamwork, resource utilization, staff and self‑development as outlined in performance review.
- Performs other duties as assigned or directed to ensure smooth operation of the department/unit.
- Demonstrates competency in caring for neuro patients, including airway management, tracheostomy care, and assisting physicians during neuro procedures.
- Performs pharyngeal, endotracheal, and tracheostomy suctioning according to established policy and procedure.
- Uses equipment and cares for patients with a halo brace.
- Manages ventilator‑dependent patient interventions.

Qualifications- Graduate of an accredited school of nursing; BSN preferred.
- Proficient in using computer software, including electronic patient records.
- Displays ability to use the internet for literature searches regarding patient care.
- Valid Connecticut registered nurse license.
- BLS required.
