RN Visiting Nurse Case Manager
Masonicare Home Health & Hospice - Trumbull, CT area
Day Shift / 40hrs/wk
OverviewMasonicare Home Health & Hospice, an affiliate of Masonicare, is seeking a dedicated and accomplished Registered Nurse Case Manager local to Trumbull, CT area.
Responsibilities- Ensures quality and safe delivery of nursing services to patients in the home setting.
- Conducts visits primarily during the day shift with flexible scheduling; weekend rotation may be required (typically one weekend per month or less).
- Develops, implements, and evaluates the plan of care, making necessary revisions to address identified problems.
- Coordinates care, maintaining continuity with other health professionals, identifies interdisciplinary care needs, and recommends additional home health services.
- Supervises Licensed Practical Nurses and Home Health Aides, delegating duties and conducting supervisory visits per regulatory guidelines.
- Maintains accurate electronic documentation to facilitate OASIS data set generation, orders, timesheet creation, and other required reports.
- Minimum 1 year of clinical nursing experience.
- New graduates will not be considered.
- Previous home‑care experience is desired but not required; on‑the‑job training will be provided.
