About This Role

The Outpatient Pediatric Hemophilia Nurse Coordinator is a Registered Nurse responsible for coordinating patient care across the continuum within Pediatric Treatment Centers. This role is ideal for someone who excels in clinical practice, care coordination, and multidisciplinary collaboration.

  Trumbull, CT

• Coordinate all aspects of patient care for the specific specialty.
• Provide patient and family education on medical conditions and treatment regimens.
• Collaborate with the multidisciplinary team to ensure seamless patient care.
• Participate in data collection related to clinical and operational efficiencies.

• Education: Bachelor's Degree in Nursing preferred, Connecticut RN license.
• Experience: 2‑3 years' experience in nursing with knowledge of infusion therapy. Experience in an ambulatory setting preferred.

  Licensed as a Registered Nurse from the State of Connecticut. APHON certification preferred but not required.
