• Serve as a case manager for your assigned individuals, helping to guide their goals, supports, and daily needs
• Coordinate services, appointments, and plans of care; ensure follow-through on all recommendations
• Build strong relationships with families, guardians, and interdisciplinary team members
• Support individuals in community activities, recreational programs, and daily living routines
• Maintain clear and accurate documentation, including program data, notes, billing, and medical information

• High school diploma or equivalent
• Valid driver’s license and reliable vehicle, ability to possess a State of CT Public Service License
• Attention to detail
• Compassion, patience, flexibility, and sound judgment
• Ability to lift up to 50 pounds
