Overview
Serving as a Home Health Aide CNA for hospice patients requires a uniquely compassionate person—someone who can manage daily care needs while providing a sense of comfort, stability, and normalcy within the patient’s home. Practically, this role means working as part of the Hospice Interdisciplinary Group under the supervision of a registered nurse or licensed practical nurse. On a human level, it means honoring each patient with the dignity, empathy, and respect they deserve.
Legacy Meridian Park Medical Center is the South Metro area’s full-service medical center, with the friendly feel of a community hospital and the skill and services of a much larger medical center. Among our specialties: stroke, chest pain, minimally invasive and robotic surgery, cancer care and orthopedic and total joint surgery. Plus, a birth center and breast health center.
Responsibilities- Provides care delegated and supervised by a registered nurse to meet the needs of clients determined both initially and at two-week intervals.
- Participates in the development and implementation of the plan of care as delegated.
- Organizes and adapts plan of care with consideration for patient/client and family needs, desires and routines.
- Demonstrates awareness and reinforces priorities in the plan of care.
- Encourages self-care and independence as appropriate.
- Identifies normal and abnormal mental function and demonstrates skills for addressing the needs and behaviors of the cognitively impaired.
- Cares for and maintains client’s environment by avoiding environmental hazards, using restraints properly, and consistently using infection control procedures and barriers.
- Recognizes abnormal signs and symptoms of common disease conditions and communicates accurately and with timeliness.
- Provides and maintains basic nutrition and oral fluid intake.
- Recognizes emergencies and responds demonstrating knowledge of emergency procedures.
- Completes documentation according to guidelines.
Education:
- Completion of a Board approved nursing assistant training program.
- Current CNA and ability to meet state Home Health Aide Certification required.
- Current CPR certificate required.
- Valid driver’s license required.
USD $21.25 - USD $30.39 /Hr.
