Legacy Health is seeking a Certified Wound Ostomy Nurse to provide expert wound, ostomy, and continence care across our Portland-based system. In this role, you'll perform advanced assessments, develop evidence-based treatment plans, and collaborate with physicians and bedside teams to promote optimal healing and skin integrity. You will educate patients, families, and staff, serve as a consultant for complex cases, and support quality initiatives that advance patient-centered care.

Responsibilities

  • Provide specialized wound, ostomy, and continence assessments and evidence-based treatment plans for diverse patient populations
  • Collaborate with physicians, surgeons, therapists, and bedside nurses to optimize skin integrity and healing outcomes
  • Educate patients and families on ostomy care, wound management, and prevention strategies to support safe discharge and self-management
  • Develop and deliver staff education, competencies, and best-practice guidelines on wound and ostomy care across units
  • Document assessments, care plans, interventions, and progress in the electronic health record in accordance with regulatory standards
  • Participate in multidisciplinary rounds, quality initiatives, and product evaluations to advance clinical practice and patient safety
  • Serve as a clinical resource and consultant for complex wound and ostomy cases across Legacy Health facilities
  • Contribute to policy development, pathway design, and performance improvement projects related to skin and ostomy care
