Coordinates patient care in the Emergency Department to ensure timely placement at the appropriate level of care. Collaborates with patients, families, physicians, and interdisciplinary teams to assess medical necessity, determine admission status, facilitate care, and support safe discharge planning. Provides care coordination for patients with complex medical and mental health needs across the continuum to promote efficient, high-quality outcomes.

United Staffing Solutions Job . Pay package is based on 10 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
