Overview
Registered Nurse – Case Management position in a 5x8 day shift, travel assignment located in Tuba City, AZ.
Start date:
09/07/2026 for a 13-week assignment. Weekly pay: $2,258 to $2,379. This role supports outpatient and inpatient case management within a surgery center setting and related care coordination activities. No guaranteed hours are stated for this assignment.

09/07/2026 - Travel assignment in a surgical care setting with transitional care responsibilities
- Serve as Outpatient/Inpatient Nurse Case Manager, coordinating care for high-risk patients across specialty clinics
- Assess care needs, develop service plans, coordinate resources, monitor progress, and respond to emerging client needs
- Facilitate transitions from admission through discharge, ensuring continuity of care and appropriate post-hospital referrals
- Evaluate ED cases for medical necessity and care coordination needs; work with clinicians to determine admission status
- Propose and implement outpatient plans for safe discharge with physician approval; collaborate with multidisciplinary teams
- Education:
Associate’s or Bachelor’s Degree in Nursing - Licensure:
Valid, current, full, and unrestricted Registered Nurse license to practice in the U.S. or a U.S. territory - Certification:
Basic Life Support (BLS) certification from the American Heart Association - Experience:
at least 3 years of clinical nursing experience, including 1 year in an outpatient/inpatient setting; demonstrated knowledge of case management, contract health, and utilization review processes - Background:
Fingerprinting and background clearance required prior to start; ability to work in a fast-paced, diverse clinical environment

Shared contractor housing available; hospital rental properties are very limited; no pets or family allowed per policy - Housing process:
