Complex Care Solutions (CCS) is seeking a Nurse Practitioner for per diem work to conduct in-home assessments across Henrico County, VA. Providers will spend up to an hour with members to capture comprehensive needs and plan care to keep patients at home and out of the hospital.
Responsibilities include documentation, adherence to CMS/Government guidelines, and coordination with PCPs and health plans. Telehealth options may be available; in-home visits rate applies where permitted.
