Home Health Registered Nurse

Position Overview

As a Center Well Home Health nurse reporting to the Clinical Manager, you will provide personalized care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients return to the life they love.

• Provide skilled nursing care to patients onsite in their homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to patients' physician and the Clinical Manager.
• Oversee the work of Home Health Aides and ensure appropriate completion of documentation.

• Med surg, ICU, ER, or acute nursing experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance, and reliable transportation.

• One year nursing experience.
• Home Health experience.

• Scheduled weekly hours: 40 hours per week.
• Pay range: $77,200 - $106,200 per year.

• Driver safety program: requires valid state driver’s license and vehicle insurance.
