Join our team as a Nurse Practitioner dedicated to serving high‑risk, high‑cost Medicaid and AHCCCS members in their homes to improve health outcomes and quality of life.
Responsibilities- Examine patients, review diagnostic tests, create treatment plans, prescribe medications, and educate families.
- Coordinate care with providers and manage care gaps.
- Work closely with a multidisciplinary team to engage members, support transitional care, and address barriers within their environment.
- Typically see 5 patients daily and manage a caseload of 100 members monthly.
- Proven clinical experience as an NP.
- Strong communication skills and ability to connect with diverse populations.
- Valid NP license in Arizona.
- Preferred language skills in Spanish.
- Coverage area:
Yavapai East. - Salary: starting at $130,000, plus insurance, retirement plans, bonuses, and flexible scheduling.
