Job Summary
Under the direction of the Public Health Director, the Public Health Manager for Chronic Disease Prevention leads the development, implementation, and evaluation of culturally responsive programs aimed at reducing the burden of chronic diseases in tribal communities. This role promotes health equity through education, policy development, and interdepartmental collaboration, with a focus on diabetes, heart disease, obesity, and related conditions.
Principle Duties and Responsibilities Program Leadership & Development- Design and manage chronic disease prevention initiatives that integrate traditional healing, community voice, and evidence-based practices.
- Collaborate with tribal departments, schools, and health providers to deliver culturally grounded health education and screenings.
- Coordinate with tribal departments, behavioral health, education, justice, and cultural programs to align services.
- Develop community presentations and outreach activities in partnership with the other tribal programs to share historical and cultural perspectives on chronic disease.
- Facilitate outreach campaigns and workshops focused on nutrition, physical activity, tobacco cessation, and stress management.
- Engage youth in prevention messaging through peer‑led initiatives that connect chronic disease risks with behavioral health and self‑esteem.
- Coordinate with tribal elders and cultural leaders to ensure messaging is respectful and relevant.
- Promote culturally grounded wellness strategies and traditional healing practices.
- Participate in interagency collaborations and represent the program in local, state, and national forums.
- Oversee program budgets, including planning, monitoring, and reporting expenditures in compliance with tribal, state, and federal guidelines.
- Manage grant funding and ensure timely submission of financial reports and renewal applications.
- Prepare reports, grant applications, and policy documents to support program sustainability and growth.
- Coordinate procurement, contracts, and vendor relationships for program‑related services and supplies.
- Supervise staff and ensure efficient operations, documentation, and recordkeeping.
- Supervise CDP program staff, ensuring efficient operations, training, performance reviews, recordkeeping, and adherence to culturally competent standards of care.
- Support professional development and capacity building for team members and community health workers.
- Collect and analyze health data to identify gaps, barriers, and opportunities for improvement.
- Lead continuous quality improvement efforts and ensure data‑informed decision‑making.
- Report outcomes to tribal leadership and funding agencies.
- Track and analyze programmatic procedures and outcomes to ensure effectiveness and compliance.
- Provide research, analysis, and recommendations to leadership on service gaps and training needs.
- Develop and maintain documentation, reports, and scopes of work for grant compliance and strategic planning.
Perform other duties of a similar nature or level as requested by the Supervisor or Director.
Knowledge,Skills and Abilities
- Chronic disease prevention strategies, including evidence-based approaches to reduce the incidence of diabetes, heart disease, obesity, and related conditions.
- Tribal health systems, governance structures, and culturally responsive care models that integrate traditional healing and community voice.
- Public health program planning, implementation, and evaluation methodologies.
- Health promotion techniques, including nutrition education, physical activity programs, tobacco cessation, and stress management.
- Behavioral health intersections with chronic disease, including trauma-informed care and mental wellness strategies.
- Federal, state, and tribal regulations related to public health, chronic disease management, and confidentiality (e.g., HIPAA).
- Community engagement and outreach strategies, especially those tailored to tribal elders, youth, and families.
