Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
Job SummaryA Lead Navigator is an individual in recovery from AOD use and/or GMH for at least one year and has at least 90 days experience as a Navigator or Peer Support Specialist. The Lead Navigator works with homeless individuals, providing outreach and supportive services to assist them in ending their homelessness. The Lead Navigator serves as the bridge for the patient, coordinating and monitoring behavioral health and non‑behavioral health community‑based services, and facilitates delivery of these services by supporting the individual.
The Lead Navigator conducts outreach, engagement, case management, and ongoing wrap‑around supportive services, mentors Navigator II staff, and coordinates care with community providers and stakeholders.
- High school diploma or GED required. Associate degree or higher in a related field preferred but not required.
- Minimum of one year of recovery from substance use and/or mental health disorders required.
- 1–3 years of full‑time health‑care related work experience preferred.
- 1–3 years of substance‑abuse/mental‑health recovery‑oriented service preferred.
- Equivalent combination of education and experience accepted.
- Current AZ driver’s license (valid and in good standing).
- Clear 39‑month motor vehicle record.
- Behavioral Health Technician certification in accordance with A.A.C. R9‑10‑115 and CBI policies.
- Arizona fingerprint clearance card, site specific.
