The Health & Housing Navigator will recruit clients into a multi-pronged intervention consisting of sexual health, hepatitis, and substance misuse screening; brief interventions and education sessions; linkage to biomedical and behavioral interventions; and supportive services. The program will use social media campaigns to promote awareness of mental health issues, risk reduction strategies, substance use, hepatitis, and HIV prevention options, and anti-stigma messaging.
Responsibilities- Perform initial comprehensive case management assessment and develop a plan of intervention that addresses housing needs and care coordination.
- Assist a minimum of 75 clients with housing applications such as Section 8 and 2010e.
- Work with Program Coordinator and Mental Health Worker to develop individualized housing plans and assist unstably housed individuals with obtaining long‑term housing.
- Provide education, case management, and benefits navigation to target population; provide linkages for PrEP, PEP, ART/HIV or STI treatment, and assist with SDOH.
- Provide individual and group education on topics such as housing retention, tenancy skills, and money management.
- Manage day‑to‑day Drop‑in Center/office activities that incorporate social, education, and community building activities.
- Ensure all data and reports are submitted by 12 p.m. every Friday.
- Other duties and responsibilities as assigned.
- High School Diploma or GED, and one year of applicable experience and/or equivalent experience.
Minimum one (1) year of similar work experience, facilitating groups and performing outreach/community engagement activities.
Qualifications- Previous professional or personal experience in housing placement and assistance preferred.
- Commitment and knowledge of issues related to homelessness, mental health, and substance misuse.
- Previous work experience with high risk, YMSM, HIV+ populations.
- Outstanding writing and communication skills.
- Certified in HIV/HCV testing or will obtain certification within 60 days of hire.
- Must possess a valid NYS driver’s license.
Compensation: $40,000.00 – $45,000.00 per year
