Comply with all state-mandated Physical Therapy Practice Act and other legal mandates.

Evaluate client for therapy needs and provide written orders/treatment plan for services.

Ensure plan of care is consistent with client assessment and needs.

Instruct other personnel and/or family members and caregivers on intervention goals.

Report client’s response to treatment or changes in condition to DON, ADON and/or client’s primary provider.

Provide in-service education for agency personnel, as needed.

Current unrestricted Physical Therapist license in state for which care is being provided

One (1) year experience as physical therapist, preferably in an in-home setting

CPR Certification, required

Two-step negative TB skin test or chest x-ray, required.

Valid Driver’s License and auto insurance, required

Understands regulatory compliance with state and federal regulations

Adheres to HIPAA and maintains client confidentiality
