Center Well Home Health is seeking a licensed Occupational Therapist to assess patients in their homes, develop retraining plans and work with physicians to implement the Plan of Care. You will help patients regain independence through targeted rehabilitation and functional improvement.
Responsibilities include therapy to restore strength, range of motion and coordination, documenting progress, and participating in care coordination and discharge planning.
