Registered Nurse- Green Valley, AZ

Haven Home Health is seeking a compassionate and experienced Registered Nurse (RN) to join our team on a PRN basis in Tucson, AZ, covering the Green Valley area.

As a PRN Home Health RN, you will provide skilled nursing care to patients in the comfort of their homes. You will work independently while being supported by a collaborative and responsive clinical team.

• Perform skilled nursing visits in patients’ homes
• Conduct comprehensive assessments (including OASIS when applicable)
• Develop and implement individualized care plans
• Provide patient and caregiver education
• Coordinate care with physicians and interdisciplinary team members
• Complete timely and accurate documentation

• Current and active Arizona RN license
• Minimum 1 year of home health experience preferred
  
  OASIS experience preferred (willing to train the right candidate)
• Strong clinical assessment skills
• Excellent communication and organizational abilities
• Current CPR certification
• Valid driver’s license and reliable transportation
