The Memory Care Unit, Registered Nurse is responsible for coordinating Resident treatments with Physical, Occupational and Speech therapy. The Incumbent reports and charts significant changes in resident condition, evidence of pain or motivational level onto VHA's electronic health systems applications. Duties include caring for dementia patients related behaviors, the RN collaborates with interdisciplinary care teams and family relatives as well as assist with restorative nursing care.
The Memory Care Unit, Registered Nurse assesses, plans, implements, and evaluates the effectiveness of nursing care delivered to the patients. The incumbent adheres to the established policy and procedures and the systemic clinical changes. The incumbent will be responsible for overseeing and completing the required assessments and documentation for the effective management of residents covered under Community Care Living Center, Memory Care Unit and will monitor residents to ensure targeted discharge, education and home care needs are met.
Handles routine admissions and discharges.
Duties May Include but Not Limited To:
* The incumbent will monitor vital signs.
* Perform intravenous therapy, ostomy care and tracheotomy care.
* Administer enteral tube feedings
* Perform wound care management.
* Help residents with range of motion (ROM) exercises.
* Administer indwelling catheter care.
* Monitor respiratory care.
* Perform Cardiopulmonary resuscitation (CPR).
* Administer medication.
