RN Case Manager

We are expanding our Home Health and Hospice Nursing Teams in Tucson, AZ. This part‑time position (2 days during the week and weekend on‑call shifts) offers a flexible schedule and an hourly compensation range of $38–$44.

• Status:
  Part‑time – 2 weekdays and weekend on‑call shifts (Friday 5 pm – Monday 8 am, 2 weekends a month)
• Compensation: $38–$44 per hour

• Conduct comprehensive physical, social, and psychological assessments.
• Collaborate with patients, families, and the interdisciplinary group to create individualized care plans.
• Manage care delivery and maintain meticulous patient records.
• Act as a resource for families, making proactive referrals, and fostering a holistic care experience.

• Active Registered Nurse (RN) license in Arizona.
• Previous Home Health or Hospice experience (preferred).
• Effective verbal, written, and time‑management skills.
• Valid driver’s license, reliable transportation, and proof of current auto insurance.
• Ability to perform patient transfers with a full range of movement.
• Current CPR certification and negative TB test (assistance provided).
• Valid AZ Fingerprint Clearance Card (assistance provided).
