Position: LPN Days and Nights Need for State of Arizona

Schedule

Working 12‑hour shifts: 6:30 a.m. – 6:30 p.m. (days) and 6:30 p.m. – 6:30 a.m. (nights) for a total of 36–40 hours per week.

Job Overview

This is a contracted position for the State of Arizona.

Responsibilities
  • Administer medication and perform treatments on an assigned unit, as ordered by the physician and within the scope of license.
  • Receive and deliver a variety of information regarding residents’ medical, social, functional and other needs.
  • Participate in receiving and giving of shift reports.
  • Communicate with residents, family and staff to ensure the resident receives appropriate care from the interdisciplinary team.
  • Perform the initial admission nursing assessment on newly admitted residents; assist with the development of the Minimum Data Set (MDS) assessments, ongoing quarterly reviews and care plans.
  • Maintain appropriate clinical and administrative documentation regarding diagnosis, assessments, treatment and summary of client progress.
  • Lead and perform limited supervision of subordinate staff; assist as needed with direct resident care for Activities of Daily Living (ADLs) including bathing, dressing, eating, toileting, personal hygiene and mobility.
  • Conduct and participate in in‑service training and staff development programs for regularly employed staff.
  • Utilize AMS concepts and tools for problem solving and continuous work/process improvements and efficiency; perform other duties as assigned.
  • Apply knowledge of gerontological/geriatric nursing theory and practices; monitor residents’ conditions and changing medical/physical/psychological care needs.
  • Apply and interpret state and agency laws and regulations governing geriatric nursing practices; develop and carry out patient care plans within a team nursing concept.
  • Effectively communicate orally and in writing; build interpersonal relationships.
  • Learn, implement and monitor AMS methods and use AMS concepts and tools for continuous improvement and efficiency.
Qualifications
  • Active RN license in the state of Arizona.
  • Basic Life Support (BLS) certification.
  • COVID‑19 vaccination or documented declination.
  • Hepatitis B vaccination or declared declination.
  • Negative tuberculosis (TB) skin test.
Preferences
  • Prefer at least one year of experience in a skilled nursing facility or long‑term care setting.
