Job Summary
The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
Responsibilities- Complete initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs.
- Provide complete physical assessment and history of current and previous illnesses.
- Provide professional nursing care by utilizing all elements of the nursing process.
- Assess and evaluate patient status, writing and initiating plans of care, re‑evaluating regularly, and revising as necessary.
- Use health assessment data to determine nursing diagnosis and develop a care plan with goals, incorporating palliative nursing actions.
- Include patient and family in planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed by physician.
- Counsel patient and family in meeting nursing and related needs.
- Provide health care instructions per assessment and plan.
- Assist patient with activities of daily living and facilitate self‑sufficiency or comfortable care.
- Act as Case Manager, coordinating care for assigned caseload.
- Maintain accurate clinical notes and pain/symptom management records.
- Communicate with physician, community health persons, and interdisciplinary team.
- Teach self‑care techniques; provide medication, diet instructions.
- Provide safe environment and continuity of care.
- Attend interdisciplinary group meetings.
- Participate in on‑call duties, ensure equipment and services availability, supervise ancillary personnel, delegate responsibilities.
- Maintain professional knowledge through continuing education.
- Participate in quality assessment, performance improvement teams.
- Graduate of a National League for Nursing accredited school of nursing.
- Minimum of two years experience, at least one year in public health, home care, or hospice nursing preferred.
- Registered nurse with current licensure in the state.
- Experience supervising hospice aides; management experience not required.
- Excellent observation, problem‑solving, verbal, and written communication skills; nursing skills per competency checklist.
- Ability to organize and prioritize workload independently.
- Physically able to walk, stand, lift, position, and transfer patients, handle supplies, and perform required tasks.
- Visual acuity and hearing adequate for nursing skills.
- Current CPR certification.
- Valid driver’s license and automobile with insurance; ability to travel to community partners and patients.
- Self‑directed and capable of working with minimal supervision.
