Licensed Practical Nurse - MAT Clinic

Overview

We are looking for a full-time LPN for our outpatient MAT clinic. Under general supervision of the Opioid Medical Director, the Psychiatric Nurse I is responsible for client care and dispensing controlled substances (Methadone, Buprenorphine, and Suboxone) to clients based on physician’s orders following careful assessment of the clients. Under general supervision of the site director, provide support to case management and medical staff.

The Psychiatric Nurse I is also expected to cooperate as a team member and provide services in accordance with client’s individual treatment plan that results in a positive behavioral change.

• Provide client care and dispense controlled substances (Methadone, Buprenorphine, Suboxone) to clients based on physician’s orders after appropriate assessment.
• Under supervision, support case management and medical staff.
• Work as a team member to deliver services in accordance with each client’s treatment plan.

• Possess a valid Arizona Level One Fingerprint Clearance Card, and:
• Currently a Licensed Practical Nurse (LPN) through the Arizona Board of Nursing.
• Must be able to obtain and maintain an Arizona Level 1 Fingerprint Clearance Card throughout employment.

• Must be 21 years of age (automobile insurance requirement) and possess a valid AZ driver’s license with a “clean” or “acceptable” driving record. An unacceptable driving record includes: more than two moving violations in the last three years; more than one at-fault accident within the last three years; or any major violation within the last three years. Submission of a 39-month driving record from DMV will be required at time of job offer.
  
  These standards must be maintained throughout employment and driving records are reviewed regularly for current employees.
