Position SummaryProvides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization. Implements and monitors patient care plans; monitors, records and communicates patient condition as appropriate. Serves as a primary coordinator of all disciplines for well‑coordinated patient care. Notes and carries out physician and nursing orders. Assesses and coordinates patient’s discharge planning needs with members of the healthcare team.

- Focused assessment based on patient condition, setting and population
Problem identification is the nurse’s clinical judgment about the patient’s response to actual or potential health conditions or needs.
Planning and Outcomes- Establishing daily goals with the patient and family
- Formulating a plan of care
- Transition or Discharge planning
- Ensures patient safety
- Medication administration
- Provision of ordered treatments
- Patient/Family education
- Provision of nursing care
- Documentation in healthcare record
Both the patient’s status and the effectiveness of the nursing care must be continuously evaluated, and the care plan modified as needed.
Care Coordination- Delegation
- Supervision
- Prioritization
- Education: Graduation from an accredited School of Nursing. Continuing education requirements as defined within the CARE program.
- Experience: One (1) year as an RN.
- Licensure/Certification s: Valid current Arizona Nursing License or compact state license and ability to become Arizona licensed in 30 days.
- Basic Life Support (BLS)
- American Heart Association and others as required by individual department as noted in Attachment
A.
- Education: Bachelor of Science in Nursing
- Licensure/Certification s: Advanced Cardiac Life Support (ACLS)
- American Heart Association
- Pediatric Advanced Life Support (PALS)
- Neonatal Resuscitation Program (NRP) or others as required by individual department.
