Position SummaryProvides direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization. Implements and monitors patient care plans. Monitors, records, and communicates patient condition as appropriate. Serves as a primary coordinator of all disciplines for well‑coordinated patient care. Notes and carries out physician and nursing orders. Assesses and coordinates patient's discharge planning needs with members of the healthcare team.

- Problem Identification
Problem identification is the nurse’s clinical judgment about the patient’s response to actual or potential health conditions or needs.
- Planning and Outcomes
Based on assessment and problem identification, the nurse sets measurable and achievable short‑and long‑range goals in collaboration with patient and family. Assessment data, problem identification, and goals are written in the patient’s care plan so that nurses and other health professionals have access to it.

Nursing care is implemented according to the care plan, ensuring continuity of care during hospitalization and in preparation for discharge. Care is documented in the patient’s record.

- Evaluation
Both the patient’s status and the effectiveness of nursing care must be continuously evaluated, and the care plan modified as needed.

Graduation from an accredited School of Nursing. Continuing education requirements as defined within the CARE program. - Experience:
One (1) year as an RN. - Licensure/
Certifications:
Valid current Arizona Nursing License or compact state license with ability to become Arizona Licensed in 30 days. Basic Life Support (BLS). American Heart Association and others as required by individual department as noted in Attachment
A.

Bachelor of Science in Nursing. - Licensure/
Certifications:
Advanced Cardiac Life Support (ACLS). American Heart Association. Pediatric Advanced Life Support (PALS). Neonatal Resuscitation Program (NRP) or others as required by individual department.
