Patient Navigator (Registry/Per Diem)

Location:

3838 N Campbell Ave, Tucson, AZ

Shift: Day, as needed to cover sick days, vacation days, and vacancies during volume surges.

Description:

The Patient Navigator works directly with patients to facilitate access to medical services, coordinate appointments, provide education about treatment options, and connect patients with community resources.

• Assesses the patient’s plan of care and develops, implements, monitors, and documents the utilization of resources and progress of the patient through their care.
• Evaluates the medical necessity and appropriateness of care, optimizing patient outcomes.
• Serves as liaison for referring physicians and assists with scheduling initial tests and consultations.
• Assists the patient/family through diagnostic services, treatment and care.
• Coordinates disease care using an interdisciplinary holistic approach, making appropriate referrals and consultations in coordination with physicians and providers.
• Ensures patient and family understand the diagnostic processes, care strategy and recommended actions.
• Evaluates patient’s functional abilities and limitations, determines if intervention is needed, establishes treatment goals, and implements a plan of care.
• Supports patient/family during difficult decision‑making periods, including end‑of‑life care and emotional support.
• Supports process improvement activities for populations of patients to achieve optimal outcomes.
• Participates in staff development and maintains current standards of practice.
• Builds collaborative relationships with physicians, payers, and other members of the health‑care team.
• Communicates pertinent information to payers and others to fulfill utilization and regulatory requirements.
• Works independently with freedom to determine how to best accomplish functions within established procedures.
• Consults with supervisor on any unusual situation.

• Bachelor’s degree in nursing or related field.
• Current RN licensure in Arizona (or PT/OT licensure if applicable).
• BLS certification required.
• Minimum 5 years of clinical experience.
• Excellent organizational, time management, communication, and writing skills.
• Proficiency in word processing and database software applications.
• Ability to teach clinical and non‑clinical personnel regarding care and diagnostics services.
• Good understanding of process improvement.
• Must maintain Level One Fingerprint Clearance Card at the time of hire for staff at Banner MD Anderson on Banner University Medical Center Phoenix campus.

• Current specialty certification.
• Prior case management experience.
• Experience with mild traumatic brain injury treatment, community outreach, or related research.
• Additional related education and/or experience preferred.
