Licensed Practical Nurse – Home Health

Schedule:

Assignment Details:

Requirements:

• Current Nursing License in the practicing state
• Valid Driver's License
• Current CPR Certification
• Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice
• Ability to use electronic medical charting and time-keeping software

Roles & Responsibilities:

• Provide skilled nursing care to patients in their residence as prescribed by physicians and in accordance with the established plan of care.
• Assist in identifying patient's physical, psycho-social, and environmental needs, and report outcomes to the supervising RN case manager.
• Participate in care planning, coordination, and implementation to ensure quality patient care.
• Educate patients and families on self-care techniques and assess responses to care.
• Maintain effective communication with patients, families, co-workers, and other healthcare professionals.
• Implement infection control measures to protect staff and patients.

Compliance / Notes:

• Must maintain professional boundaries in relationships with patients and co-workers.
• Participate in annual in-service and performance improvement programs.
• Must read, write, and speak fluent English.
• Maintain good and regular attendance.
