Registered Nurse Outpatient Hospice Case Manager

Full-time outpatient Hospice Case Manager role. Current or prior Hospice or Home Health experience is a plus. $5,000 sign-on bonus with 2+ years of related nursing experience in the field.

Coordinates patient care planning with other disciplines utilizing the nursing process and critical thinking to maintain patient safety and well-being. Some positions may be assigned to critical care areas requiring specialized training.

• Provides direct patient care, evaluates outcomes, consults with other specialists as required, and adjusts nursing care processes to ensure safe patient care.
• Writes or obtains initial nursing histories, assesses patients’ conditions, and develops individual care plans; maintains patient documentation.
• Reviews preceding shift reports and performs rounds; notes patients’ condition.
• Notifies appropriate physician regarding unusual or unexpected events requiring direct physician consultation.
• Collaborates with interdisciplinary team to reinforce patient instructional needs with patients and families, providing informational materials and noting special areas of concern.
• Evaluates outcomes of patient care and adjusts nursing plans as necessary to ensure optimal care.
• Identifies and resolves workplace health and safety issues.
• Delivers appropriate direct patient care using aseptic techniques; may manage IV lines and oversee patient movement and positioning for safety.
• May administer pharmacological vasodilator medications for nuclear myocardial perfusion imaging; complies with examination competency as defined in Imaging Services Practice Standards.
• Complies with policies, procedures, regulations, and standards; takes corrective action when violations occur.
• Orients, instructs, and educates assigned personnel.
• Behavioral Health RN responsibilities: assess psychological needs of patients and families; provide discharge and aftercare planning; coordinate with referral sources and community agencies as required.
• EP-RN Specialist responsibilities: assist physicians with EP procedures; manage catheters, electrophysiology recording systems, and quality control components.
• EP Coordinator responsibilities: oversee EP area, coordinate staffing and scheduling needs, trainings, and materials management for efficiency.
• Maternal/Child Float Pool: support coverage at multiple locations based on operational needs; aligns with TMCH values of integrity, community, compassion, and dedication.

• Education
  :
  Bachelor’s degree preferred.
• Experience
  :
  One (1) year of nursing experience in an acute care setting preferred. For EP positions, two (2) years of EP experience or demonstrated competency in EP Lab procedures.
• Licensure or Certification
  :
  Current RN licensure in the State of Arizona and Basic Life Support (BLS) certification required. Additional certification may be required per department.
• Knowledge, Skills, and Abilities
  :
  • Knowledge of direct patient care and critical care procedures and techniques.
  • Knowledge of exam indicators, contraindications, and patient preparation.
  • Clear and concise communication with staff and physicians.
  • Ability to read 12-lead EKG and interpret clinical information.
  • Ability to read and interpret safety rules, operating instructions, and procedure manuals.
  • Ability to complete routine reports and correspondence; listen and interpret instructions accurately.
  • Effective spoken communication and problem-solving skills; ability to draw valid conclusions from data.
