Role
The Registered Nurse (RN) in Home Health provides patient care, coordinates multidisciplinary team visits, and oversees episode care frequency.
Responsibilities- Perform accurate OASIS collection
- Maintain current medication profiles
- Ensure timely lab reporting
- Consult with physicians
- Participate in weekly and monthly case meetings
- Must be a licensed RN with preferred minimum one year of clinical experience
- Current nursing knowledge
