Position Summary

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and standards of professional nursing practice. The role is accountable for the quality of nursing services delivered by self or others under direction. The position utilizes specialized knowledge and nursing skills to assess data and provide appropriate care for the physical and developmental age of assigned patients.

Core

Functions

Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews history and physical with patient/family ensuring completion within appropriate timeframe. Reviews diagnostic and laboratory data, reports abnormal results to physician(s) and other caregivers, and completes assessment and reassessments according to policy.

Formulates a plan of care, including discharge plan, utilizing assessment data and patient, family and health team input. Initiates plan based on patient-specific needs, ensures goals are realistic and measurable, considers various patient needs, collaborates with multidisciplinary team, and revises plan as needed.

Implements plan of care through direct patient care, coordination, delegation and supervision of the health care team. Ensures compliance with policies, standards and regulatory requirements. Delegates appropriately, provides supervision to other staff, and promotes continuity of care through accurate communication.

Evaluates patient and family response to care and teaching, revises plan of care as needed, evaluates progress toward goals in collaboration with team, and assesses effectiveness of patient teaching.

Documents assessment, planning, implementation and evaluation in the patient record. Ensures legibility, timeliness and compliance with policy, reflecting objective/subjective data, nursing interventions and patient responses. Notes physician orders accurately.

Provides evidence‑based care, may participate in research activities within clinical practice, participates in unit or facility shared leadership, interacts in education, role development and orientation of personnel, patients, students, families and visitors, and promotes/supports growth through precepting and mentoring when appropriate.

Contributes to society through activities leading to excellent patient outcomes, participates in improvement of national nursing quality indicators and outcomes, and may participate in professional organizations.

In some roles, may supervise staff and workflow of the department.

Interacts with all levels of staff in various departments, physicians, patients, families, external contacts such as employees of other health care institutions, community providers and agencies to report and clarify information; works with clergy to provide spiritual support; synthesizes and prioritizes data from multiple sources.

Minimum Qualifications

Current, valid RN license in state of practice; temporary/interim RN license in state of practice; or compact RN licensure for current state of practice.
BLS certification required.
Additional certification or continuing education may be required based on area of practice.
For acute care: minimum one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as an RN in the specialty area.
Banner Registry and Travel acute care positions require minimum one year experience in an acute care hospital setting.
Banner Registry and Travel physician practice positions require minimum one year experience as an RN in a physician practice or acute care setting.
In home health settings: must operate personal vehicle and possess valid driver's license; auto insurance coverage must be active and maintained.
In behavioral health settings requiring fingerprint clearance: must possess appropriate Arizona fingerprint clearance card at time of hire and maintain it; also requires Arizona Criminal History Affidavit.
Additional requirements for specific roles as per hiring policies (e.g., Colorado fingerprint clearance for Banner McKee Senior Behavioral Health Inpatient Unit).

Preferred Qualifications

Bachelor's degree preferred.
Professional certification preferred.
Additional related education and/or experience preferred.
