Position Overview

As a Hospice RN Case Manager, you will coordinate and oversee all aspects of patient care for an assigned caseload. You will work closely with physicians, social workers, chaplains, hospice aides, and families to ensure patients receive the highest quality end-of-life care.

This position offers independence, flexibility, and the opportunity to provide personalized care that honors each patient's goals, wishes, and quality of life.

What You'll Do
  • Complete comprehensive patient assessments and ongoing evaluations
  • Develop, implement, and revise individualized plans of care
  • Manage pain and symptoms to maximize patient comfort and quality of life
  • Provide skilled nursing services in accordance with physician orders and hospice regulations
  • Educate patients, caregivers, and families regarding disease progression, medications, and care planning
  • Coordinate care with the interdisciplinary hospice team
  • Serve as a clinical advocate for patients and families
  • Provide emotional support and guidance during challenging times
  • Maintain accurate and timely documentation
  • Participate in interdisciplinary team meetings and care conferences
  • Ensure compliance with agency policies, Medicare regulations, and hospice standards of practice

Qualifications

Required

  • Current Arizona Registered Nurse (RN) license in good standing
  • Minimum of one year nursing experience
  • Valid driver's license, reliable transportation, and current automobile insurance
  • Strong assessment, clinical judgment, and critical thinking skills
  • Excellent communication and organizational abilities
Preferred
  • Hospice experience
  • Palliative care experience
  • Home health experience
  • Case management experience
  • Experience working independently in the field

Hospice experience is preferred but not required. We welcome nurses with home health, med-surg, oncology, ICU, case management, or other relevant clinical backgrounds who are passionate about compassionate end-of-life care.
